How do I apply?
There are several ways to apply for Medi-Cal for Children and Healthy Families. One of the easiest ways is to use the joint application for both programs. A benefit of using the joint application is that you do not have to figure out which program to apply for.
Applications go to “Single Point of Entry” for the initial screening.
- If your child is screened eligible for Healthy Families, the application will be sent to the Healthy Families Program to complete the application process.
- If your child is screened eligible for Medi-Cal, the application will be sent to your county Department of Social Services to determine eligibility and complete the application process.
When you use the joint application and your child is screened eligible for Medi-Cal, your child will receive Accelerated Enrollment (AE) which temporarily enrolls your child in no-cost Medi-Cal while the application is processed. With AE your child can begin receiving health, dental and vision care right away at no cost to you. You do not get AE when you apply at the Department of Social Services.
Call 800-880-5305 for an application or download one at www.healthyfamilies.ca.gov.
See the Appendix for documents that are required with the application.
How often do I have to reapply?
You will have to submit documentation every 12 months to see if your children can stay enrolled in the programs.
- Medi-Cal Annual Redetermination forms are sent at least 45 days before the anniversary date.
- Healthy Families Annual Eligibility Review forms are sent 60 days before the anniversary date.
If the forms are not returned with the required documentation, your children may be disenrolled from the programs and they could lose health care coverage while you complete another application.
