Child Health and Disability Prevention Program
Child Health and Disability Prevention Program (CHDP) provides free health check-ups for infants, children, teens and young adults based on periodic screening guidelines set by the federal government.
CHDP check-ups help to identify potential health problems. If your child has a problem identified during a CHDP check-up, referrals are made for appropriate treatment.
You may also take your children for CHDP check-ups when they need a school or sports physical.
CHDP does not cover doctor’s visits for illness or injury.
Who is eligible?
All children with Medi-Cal age 0 to 21
Low income, uninsured children age 0 to 19
Children enrolled in Head Start and State preschool programs
What does it cover?
CHDP offers a full range of health assessments based on the child’s age and can include:

Complete physical examination
Health and developmental history
Oral health assessment
Nutritional assessment
Behavioral assessment
Immunizations (appropriate for age)
Vision screening
Hearing screening
Lab tests (anemia, blood lead, etc.)
Health education

When potential problems are found during a CHDP check-up, referrals for treatment are made. Often children will have to receive treatment from a different doctor or clinic than the one where they had the check-up. The doctor or clinic that identifies the problem will help make the treatment appointment and provide the necessary paperwork.
