How often can my child receive CHDP?

Children are eligible to receive a regular CHDP check-up in each age category:

  • 1 month of age or less
  • 2 months of age
  • 4 months of age
  • 6 months of age
  • 9 months of age
  • 12 months of age
  • 15 months of age
  • 18 months of age
  • 2 years of age
  • 3 years of age
  • 4-5 years of age
  • 6-8 years of age
  • 9-12 years of age
  • 13-16 years of age
  • 17-20 years of age
  • From 0 to 3, your child can receive 10 health check-ups
    including immunizations that protect your child’s health!

    How much does it cost?

    There is no cost for CHDP check-ups or follow-up treatment for eligible children.

    Your CHDP provider can help you pre-enroll your children in Medi-Cal for up to two months through the “CHDP Gateway.” You can also choose to enroll your child in Medi-Cal or Healthy Families when you complete the CHDP pre-enroll and an application will be sent to you to complete. While you apply, your child receives full Medi-Cal coverage at no cost to you for additional health, dental and vision services. If your child is not eligible for no-cost Medi-Cal, you will not have to pay for the services your child received through “CHDP Gateway.”

    How do I apply?

    Call Shasta County Public Health at 800-300-5122 or 225-5122 for a list of CHDP providers. Call one of the doctors or clinics to make an appointment for a CHDP exam. You will apply for CHDP at the time of the appointment. You do not need documentation to get started.
