How much does it cost?
If your child is eligible for CCS and you take your child to an eligible CCS provider, CCS covers the cost of diagnosis and treatment of the CCS condition.
How do I apply?
If you, your doctor or another health professional, like a school nurse, think your child has a CCS eligible condition, call the local CCS office at 225-5760 for an application and assistance in completing the application.

Where do I get services?
You must take your child to a CCS eligible provider to receive CCS services or equipment. When your child’s application is approved for CCS eligible services, you will be given information about the available CCS providers.
